The referral form is not a secure channel. Please do not enter diagnoses, medication lists, test results, or other health information into it. Send those by phone, or through an authorized release once we have spoken.
Who this covers
This page explains what All in 1 Choices LLC does with information submitted through allin1choices.org. It applies to the website only. It does not describe how records are handled once someone is referred to or living in one of our homes.
What we collect
Only what you send us, and the basic records a web server keeps.
- Referral form. The organization and contact name, phone number, and email address you provide, along with the placement details you choose to add — preferred home, expected move-in date, how soon a bed is needed, the person's current location, funding source, and any notes.
- Email and phone contact. Whatever you tell us when you get in touch directly.
- Server logs. Our web host records the usual technical details of each visit: IP address, browser type, the page requested, and the time. These are kept by the host and used to keep the site running and secure.
What we do not collect
- No advertising or marketing trackers of any kind
- No third-party analytics services
- No cookies set by this site
- No profiles built about visitors, and nothing sold or rented to anyone
The site loads typefaces from Google Fonts, which means your browser makes a request to Google to fetch them. Nothing else on the page reaches a third party.
What we use it for
Referral information is used to respond to the referral: to screen for suitability, confirm bed availability, and arrange a placement. We use your contact details to reach you about that referral. We do not add referral contacts to a marketing list.
Who sees it
Referral submissions go to the staff who handle placements. We do not share them with anyone outside All in 1 Choices LLC except where the law requires it, where a court or regulator compels it, or where you have given written authorization — for example, a signed release allowing us to speak with a treatment provider or case manager.
Our website host and email provider necessarily process this information in order to deliver it to us.
How long we keep it
Referral correspondence is retained for 24 months and then deleted, unless it relates to someone who was admitted, in which case it becomes part of that person's file and follows the retention rules for resident records. Server logs are kept according to our host's standard schedule.
Security
The site is served over an encrypted connection, and the referral form posts to our own server rather than to a third-party form service. Email, however, is not an encrypted medium end to end. That is why the form asks you to keep clinical detail off it.
Your choices
You can ask us what referral information we hold about you, ask us to correct it, or ask us to delete it, and we will do so unless we are required to keep it. Write to us at the address below. Depending on where you live, you may have additional rights under state privacy law.
Children
Our homes serve adults. This website is not directed at children, and we do not knowingly collect information from anyone under 18.
Changes
If we change how we handle information, we will update this page and change the date at the top.
